Care Transitions & Discharge Planning

This intermediate-level course equips nurses (RN), occupational therapists (OT), and physical therapists (PT) with the knowledge and practical skills needed to plan, coordinate, and execute safe, patient-centered care transitions. Emphasis is placed on reducing readmissions, improving functional outcomes, addressing social determinants of health, and enhancing interdisciplinary collaboration.
